This is an important legal document. It explains the risks you are assuming by entering and using any and all facilities on, at, in connection with or related to [portion 1 of erf 157 Norwood], (hereinafter referred to as “the Facility”). It is critical that you read and understand it completely. After you have done so, please print your name legibly and sign in the space provided.
I have elected to use / make use of the Facility which will include, but may not be limited to, hairdressing and/or beautician services. I do hereby and forever release and discharge and hereby hold harmless Stoffel van Wyk, Antoinette van Wyk, their agents, employers, employees, business, associates, partners, insurers, reinsurers and/or anyone else who resides at the Facility (“the Indemnified Parties”) from any and all claims, demands, damages, rights of action or causes of action, whether present or future, arising out of or connected with my participation at the Facility, including any and all injury or loss or damage resulting therefrom or associated therewith.
I confirm that I understand and am aware that hairdressing and/or beautician services may be a hazardous activity. I also hereby confirm that I understand that hairdressing and/or beautician services involves a risk of injury and/or even death, and I further confirm that I am voluntarily, of my own will, participating in these activities and am using equipment and machinery with knowledge of the dangers associated therewith or related thereto. I might also allow a hairdresser or beautician to use equipment and machinery with the knowledge of the dangers associated therewith or telated thereto. I hereby agree to expressly assume and accept any and all risks of injury and/or death.
I do hereby further declare myself to be physically sound and suffering from no condition that would prevent my participation or use of machinery or equipment at the Facility.
I hereby acknowledge that I have thoroughly read this waiver, release and indemnity and hereby confirm that I fully understand that it is a full release of any and all liability. By signing this document, I am aware that I am waiving any right my successes or I might have to bring a legal action or assert a claim against the Indemnified Parties.
No refund on cancellations within 48 hrs of booking policy
